grinding brakes...

Lately If i am driving my Liberty down the road while the brakes are cold (feels like its coming from fronts), sometimes I get the "Grinding" feel, just like the brakes are metal to metal (reminds me - happened to my girlfriend's car when I was driving and she didn't know the brakes were metal to metal! yeah women!) but I opened up and removed the pads twice now to inspect whats going on, i cannot see whats wrong with them. The pads are Bendix ultimate and been there for 12 months now, has plenty of beef left on it.
